Food Delivery Portal
The Food Delivery Portal (FDP) is the federal system that WIC state agencies use to report on WIC vendor management activities. This system replaced The Integrity Profile (TIP) in FY 2021.

Waivers and State Plans
Waivers and State Plans (WiSP) is a federal system that allows WIC, FMNP, and SFMNP state agencies to request program waivers and manage their state plans.
- WiSP State Plan Module: Coming April 2026.
- WiSP Waiver Module: Coming soon. At this time, the Waiver Module in WiSP will not be utilized for waiver submissions. Please continue requesting waivers as previously established.

Account Creation
Access to FNS applications will require a Login.gov account that is linked to USDA eAuthentication (eAuth).
- Obtain and link a Login.gov Account.
- Click on one of the links above that begin with “SA” to begin account creation.
- Use the detailed instructions to create and link accounts.
- Verify your identity.
- Complete an FNS-674 form. More information can be found in:

- FDP accounts will require both the FNS-674 form and a SNAP STARS Confidentiality form. These forms are available from your state agency management, FNS regional office, or the FDP Helpdesk.
- Turn in the completed package to the FDP Helpdesk. You will receive an email when your user account is ready to use.
- Instructions for WiSP Users
- WiSP accounts only require an FNS-674 form. Turn in the completed package to the WiSP Helpdesk. You will receive an email when your user account is ready to use.

This notice informs the public of the annual adjustments to the reimbursement rates for meals served in the Summer Food Service Program for Children. These adjustments address changes in the Consumer Price Index, as required under the Richard B. Russell National School Lunch Act.
Instructions for FDP Users
FNS Form-674
The FNS-674 form needs to have certain data in boxes 14 – 20.
- All Users
- Box 14 System Name: Food Delivery Portal (FDP)
- Box 15 Type of Access/Role
- State Agency: FDP SA User
- Regional Office: FDP RO User
- Headquarters: FDP National Office (HQ) User
- Box 16 Action Requested: Add
- Boxes 17 and 18: Leave blank.
- Box 21 - Provide your signature, printed name and date.
- State Agency Users
- Box 19 State/Locality Codes: 2-digit state/state agency code (e.g., AZ, MP, NN).
- Please see the list of state agency abbreviation codes.
- Box 20 : copy and paste the text below, then change the state agency reference to be appropriate
- “FNCS Org (Salesforce) Environment(s): Production; Role: System generated after a State agency contact is created; License Type: Partner Community; Profile: FNSWIC_State Agency User Profile; Public Groups: FNS WIC Portal Users”
- Print out the form.
- Box 22a: Obtain your supervisor's information and ink signature.
- Scan the completed form as a PDF (or take a clear picture).
- Save the completed FNS-674 as a .PDF using the following filename format: State Abbreviation_LastName_FirstName_FNS674 (ex: MA_John_Doe_FNS674)
- Box 19 State/Locality Codes: 2-digit state/state agency code (e.g., AZ, MP, NN).
- FNS Users
- Box 20: copy and paste the text below, then add or remove the region reference (as appropriate), and add your email address as the User name:

- Box 20: copy and paste the text below, then add or remove the region reference (as appropriate), and add your email address as the User name:
Complete the STARS Confidentiality Form
All FDP users will need to complete a STARS confidentiality form as information on STARS retailers appears within FDP. This form is available from your state agency management, regional office, or the FDP Helpdesk at SM.fn.FDPHelp@usda.gov.
Turn in Completed Package
- Email the completed FNS-674 form and STARS confidentiality form to the FDP Helpdesk. The FDP Helpdesk will forward your account request package to the system’s FNS authorizing official.
- You will receive an email when your user account is ready to use.
Create and Link Login.gov User Account
Login.gov is the public’s one account and password for the U.S. government. Login.gov is a shared service and trusted by U.S. government agencies. You may visit the USDA eAuth FAQs for more information on creating your account.
Create a Login.gov account.
If a user does not already have a Login.gov account, use these steps to create an account:
- Navigate to Login.gov.
- Follow the instructions to set up your account. Review the eAuth FAQS for more information on creating your account.
- Enter a unique email address.
- Create a secure password for the account.
- Add a required multi-factor authentication option like SMS phone/text.
- You will be returned to eAuth to link your Login.gov account with eAuth. Follow the steps below to link your account.
Note: If the application you are attempting to access requires a verified identity, you will be notified and then prompted to choose the Login.gov online identity verification process or visit a Service Center Office for in-person assistance with a USDA Local Registration Authority (LRA).
Link an existing Login.gov Account
For users with new or existing Login.gov accounts please use the following steps to link your account to the USDA eAuth application:
- Navigate to the application you wish to sign into.
- Select the “Customer” option.
- Click the Login.gov option. You will be taken to the Login.gov website, where you will log in with your existing Login.gov account. This is one place that the identity verification may trigger.
- After logging in, you will be returned to eAuth to link your Login.gov account to eAuth.
- If you do not have an eAuth account, or are changing to a different business email, choose the ‘do not link to an existing eAuth account.’
- If you have an eAuth account linked to a valid business email, you may choose the ‘link to an existing eAuth account.’ This will prompt you to use your eAuth user ID and password one last time to link the two types of accounts. Once linked, you will only need your Login.gov user ID and password.
- eAuth will attempt to redirect you into the application. For more information review the eAuth FAQs for linking Login.gov.
FNS-674 Form Instructions
Once you have completed the steps to prepare your Login.gov account to connect to a USDA application, you need to complete an application package requesting a user account, for each application you wish to access.
All USDA applications use the FNS-674 form which needs to be completed and approved by an FNCS Account Manager. The form provides instructions for each field starting on its last page. Please fill out the form using your work/business information.
Each application may have additionally required forms or specific instructions for boxes in the FNS-674. Please review the specific application section of the SNAS Gateway to get the full instructions.
The FNS-674 form will need to be completed and approved before an account is created. There are certain boxes that should be filled in as noted below:
- Box 4 Work Email
- Should be the same for all USDA applications.
- Should be the email used for your Login.gov account. Login.gov will allow you to list multiple emails on your account, this box needs to match your login email.
- Should be the same for all USDA applications.
- Box 5 eAuth user ID – leave this field blank.
- Boxes 14 - 20 must match the application information. Please see the application specific instructions.
Note: The FNS-674 requires Adobe Acrobat Reader. If Adobe Acrobat Reader is needed, please download the software.
Login.gov Identity Verification
If required by the application, the user may need to complete a one-time identity verification to convert the account into a trusted account (i.e., previously known as Level 2), when logging in for the first time.
There are two methods available to verify your identity:
Verify your identity online with Login.gov.
You will need to:
- Upload a photo/scan of your driver's license or state ID card. Please review the Login.gov Accepted Identification Documents for more information. If you do not have a state issued ID you may need to verify your identity in person.
- Enter your Social Security number.
- Enter your primary phone number. If Login.gov can’t verify the phone number, your address can be verified by mail instead. This takes approximately 5-10 business days.
- Once your identity is verified, re-enter your Login.gov password. Take note of your personal key and enter on the next page.
- Once the process is complete, click “Agree and Continue” to be redirected back to the application.
