FNS-674 Form Instructions

Once you have completed the steps to prepare your Login.gov account to connect to a USDA application, you need to complete an application package requesting a user account, for each application you wish to access.

All USDA applications use the FNS-674 form which needs to be completed and approved by an FNCS Account Manager. The form provides instructions for each field starting on its last page. Please fill out the form using your work/business information.

Each application may have additionally required forms or specific instructions for boxes in the FNS-674. Please review the specific application section of the SNAS Gateway to get the full instructions.

The FNS-674 form will need to be completed and approved before an account is created. There are certain boxes that should be filled in as noted below:

• Box 4 Work Email
  • Should be the same for all USDA applications.
  • Should be the email used for your Login.gov account. Login.gov will allow you to list multiple emails on your account, this box needs to match your login email.
• Box 5 eAuth user ID – leave this field blank.
• Boxes 14 - 20 must match the application information. Please see the application specific instructions.

Note: The FNS-674 requires Adobe Acrobat Reader. If Adobe Acrobat Reader is needed, please download the software.
