Resource | Federal Register Notices Food Distribution Program: Value of Donated Foods (July 1, 2020 - June 30, 2021)

This notice announces the national average value of donated foods or, where applicable, cash in lieu of donated foods, to be provided in school year 2021 (July 1, 2020 through June 30, 2021) for each lunch served by schools participating in the National School Lunch Program, and for each lunch and supper served by institutions participating in the Child and Adult Care Food Program.

Resource | Federal Register Notices CACFP: National Average Payment Rates, Day Care Home Food Service Payment Rates, and Administrative Reimbursement Rates for Sponsoring Organizations of Day Care Homes (July 1, 2020 - June 30, 2021)

This notice announces the annual adjustments to the national average payment rates for meals and snacks served in child care centers, outside-school-hours care centers, at-risk afterschool care centers, and adult day care centers; the food service payment rates for meals and snacks served in day care homes; and the administrative reimbursement rates for sponsoring organizations of day care homes, to reflect changes in the Consumer Price Index.

Resource | Federal Register Notices National School Lunch, Special Milk, and School Breakfast Programs; National Average Payments/Maximum Reimbursement Rates (July 1, 2020 - June 30, 2021)

This notice announces the annual adjustments to the national average payments, the amount of money the Federal Government provides states for lunches, afterschool snacks, and breakfasts served to children participating in the National School Lunch and School Breakfast Programs; to the maximum reimbursement rates, the maximum per lunch rate from federal funds that a state can provide a school food authority for lunches served to children participating in the National School Lunch Program; and to the rate of reimbursement for a half-pint of milk served to non-needy children in a school or institution that participates in the Special Milk Program for Children.

The nutrition educator from the Lac du Flambeau Band of Lake Superior Chippewa Indians in Wisconsin has compiled a cookbook containing healthy recipes that utilize USDA Foods found in the FDPIR food package.

FNS Form-674

The FNS-674 form needs to have certain data in boxes 14 – 22.

All Users
  • Box 14: Waivers and State Plans (WiSP)
  • Box 15: Non-Privileged
  • Box 16: Add
  • Boxes 17 and 18 - Leave blank.
State Agency Users
  • Box 19 State/Locality Codes: 2-digit state/state agency code (e.g., AZ, MP, NN).
  • Box 20: copy and paste the text below:
    • “FNCS Org (Salesforce) Environment(s): Production, UAT; Role: System generated after a State agency contact is created; License Type: Customer Community Plus; Profile: State Agent WISP; Public Groups: "WISP State Agency Users” Permission Sets: WISP State Agent Permissions”
  • Print out the form
  • Box 22a: Obtain your supervisor's information and ink signature.
  • Scan the completed form as a PDF (or take a clear picture).
  • Save the completed FNS-674 as a PDF using the following filename format: State Abbreviation_LastName_FirstName_FNS674 (ex: MA_John_Doe_FNS674).
FNS Regional Users
  • Box 19: Set to Region code (e.g., NERO, WRO)
  • Box 20: copy and paste the text below, then add the region reference:

Turn in Completed Package

  1. Email the completed FNS-674 form to the WiSP Helpdesk at SM.fn.WiSPHelp@usda.gov. The WiSP Helpdesk will forward your account request package to the system’s FNS authorizing official.
  2. You will receive an email when your user account is ready to use.
