The information collected via this form will be reviewed and approved by FNS for possible inclusion in the SNAP-Ed Library. These collected resources may be shared with SNAP-Ed state and local agencies to provide technical assistance, best practices and help enhance program activities.

This memo provides reporting guidance for the FNS-640 report beginning with reporting for school year (SY) 2018-19 due March 1, 2020.

For FY 24, states, territories and ITOs must clearly attest that non-federal funds have been identified and are available to meet the 50 percent match requirement. 

The FNS announces the "Farm to School State Formula Grant" established by the American Rescue Plan Act.

SNAP state agencies must establish procedures to screen for and apply the general work requirements and ABAWD work requirements and time limit. The SNAP Work Rules Screening Checklists and Flow Chart were developed to assist SNAP state agency staff in determining if an individual is subject to any of the SNAP work requirements.

This information, effective as of FY 2024 case reviews, is being collected to assist the Food and Nutrition Service with SNAP's Quality Control Reviews. This is a mandatory collection and FNS uses the information for program monitoring, evaluation, corrective action, and characteristics.

FNS Instruction 796-2, Revision 4: Financial Management - CACFP 

This report is required under provisions of 7 CFR 275.14 (SNAP). This information is needed for the review of state performance in determining the eligibility of applicants and recipients. The information is used to determine state compliance, and failure to report may result in a finding of non-compliance.

This report, effective as of FY 2023 case reviews, is required under provisions of 7 CFR 275.24 (SNAP). This information is needed for the review of state performance in determining recipient eligibility.
