This notice invites the general public and other public agencies to comment on this proposed revision to the currently approved information collection for the Supplemental Nutrition Assistance Program (SNAP) form FNS–889, “SNAP Education (SNAP-Ed) Connection Resource Sharing Form.”

Collection of this information will increase FNS' ability to formulate nutrition education interventions that resonate with the intended target population, particularly low-income families.

In order for FNS to monitor the effectiveness of E&T programs state agencies are required to report outcome data on five separate reporting measures.
