This is a new collection for the Healthy Meals Incentives Recognition Awards Application. The Recognition Awards will recognize school food authorities (SFAs) that have made significant improvements to the nutritional quality of their school meals by exceeding the transitional school meal pattern requirements, engaging students, and implementing innovative practices. SFAs can apply to one or more Recognition Awards included in the application form.
FNS issued regulation that prescribe the methods for making payments of funds to state agencies to use for administrative expenses incurred in overseeing and providing technical assistance in connection with activities undertaken under the NSLP, the SMP, the SBP, the CACFP, and the FDP. For this revision, FNS has revised the FNS-525 form to eliminate data fields which collect data that can be obtained from the FNS financial data systems.
This submission is a revision of a currently approved collection which covers the reporting and recordkeeping burden associated with the Seniors Farmers' Market Nutrition Program. The information collected is used by USDA to manage, plan, evaluate, make decisions, and report on SFMNP program operations.
This request for approval of information collection is necessary to obtain input into the development of nutrition education interventions for population groups served by FNS. Collection of this information will increase FNS' ability to formulate nutrition education interventions that resonate with the intended target population, particularly low-income families.
Households in which members receive substantial lottery and gambling winnings are ineligible for SNAP until they meet allowable financial resources and income eligibility requirements. States are also required to work cooperatively with entities responsible for gaming in their state to identify individuals and households with substantial winnings. SNAP households must report substantial winnings to state SNAP agencies.
This is an extension, without change, of a currently approved collection. USDA requires that state agencies report outcome data for the SNAP E&T programs. In order for FNS to monitor the effectiveness of E&T programs state agencies are required to report outcome data on five separate reporting measures.
FNS is delaying the effective date for the interim final rule - SNAP: Requirement for Interstate Data Matching to Prevent Duplicate Issuances - until Dec. 6, 2022.
This information collection addresses the mandatory state agency information and burden estimates associated with the following state agency options under SNAP: establishing and reviewing standard utility allowances and establishing methodology for offsetting cost of producing self-employment income.
The FNS-380-1, Quality Control Review Schedule, was developed by FNS for state use to collect both QC data and case characteristics for SNAP and to serve as the comprehensive data entry form for SNAP QC reviews. State agencies are required to perform QC reviews for SNAP. The information needed to complete this form is obtained from the SNAP case record through state agency quality control findings. The information is used to monitor and reduce errors, develop policy strategies, and analyze household characteristic data.
This is an ongoing, voluntary information collection which is used by FNS to update the USDA National Hunger Clearinghouse. FNS collects information about the summer meal sites from state agencies using FNS-905. This form collects the site name, location, and operating details about the summer meal sites, such as dates and times of site operation.
