This memo provides questions and answers addressing the FY 2024 benefit issuance and participation reporting requirements for Summer EBT. The guidance is divided into three sections. The first section addresses the benefit issuance reporting requirements for Summer EBT; the second section addresses participation reporting requirements; and the third section provides special instructions for reporting Summer EBT benefits issuance and participation data when Summer 2024 benefits are issued after Sept. 30, 2024 (i.e., in FY 2025).
FNS requires that inventories do not exceed a six-month supply of any USDA Foods without approval from FNS. Per 7 CFR 250.17(a), state agencies administering TEFAP are required to use Form FNS-155, Inventory Management Register, to report any food items in state and state-contracted warehouses that exceed six months of inventory.
The ITO/state agency must submit Form FNS-152, Monthly Distribution of Donated Foods to Family Units, that documents the data obtained in the physical inventory with the perpetual inventory record. Shortages must be reported to the appropriate FNS regional office.
Form FNS-101: Participation in Food Programs - by Race
Form FNS-667 is used by state agencies to report use of TEFAP administrative funds.
Form FNS-57 is used by consignees to report overages and shortages in a shipment and commodities damaged in transit.
State agencies apply for reimbursement using Form FSA-21, submitted to the appropriate FNS regional office for approval.
