Grant Modification Request Template (24.67 KB)
This template is intended for all active Farm to School grantees requesting a budget modification for their projects. Grantees can use this template to clearly identify requested line-item changes and demonstrate their impact on the project budget.
Completed modification request templates should be submitted to the grantee's regional specialist and/or grant specialist.
