What to Have for Your WIC Appointment

You will need to have several documents for your WIC appointment. Please ask your [[location-name]] which documents are required. Below are examples of acceptable documentation:

  • Proof of identity (such as a driver’s license or state ID, passport, or birth certificate, employer or school ID, health benefits card, or crib card) for each person who will be enrolling in WIC.
