Farm to School Grantee Reports

These documents are the blank fillable PDFs for the baseline report and final report for Farm to School grantees. These reports are intended for active Farm to School grantees only.

Instructions to ensure file opens properly:

1. Right click the document link and choose "save link as" to save the file to your computer.
2. Open Adobe Acrobat.
3. From within Adobe Acrobat, choose "File" and "Open" and navigate to your saved pdf.
4. When prompted choose "Enable All Content" and the report should open.

These reports will not open in the web browser and you will receive an error message if you do not follow the steps above.
